SERVICES
Dr. Raque is a Blue Cross/Blue Shield Preferred Provider and a Medicare Participating Physician. Our services cover a complete range of dermatological care including skin examination, treatment of skin diseases, skin cancer, skin surgery, sclerotherapy, superficial skin peels, radiation therapy and ultra-violet light treatment for psoriasis (PUVA, UVA & UVB).
Patients are encouraged to call with any questions they have on medical problems. The office staff have been trained to answer most questions. When requested, the doctor will return your call at his earliest opportunity.
PRESCRIPTIONS
Patients are given refills on their prescriptions to last until their next visit. Prescription renewals will not be authorized for a period greater than six months without an appointment for evaluation of the continued need for medication. Some medications may require more frequent evaluation.
IMPORTANT INSURANCE AND PAYMENT INFORMATION
For regular office visits, it is customary to pay for services at the time they are rendered. The receipt form we give you at the end of each visit contains the information needed to process your insurance claim, if we are not a provider for your insurance plan.
It is the policy of this office to accept Medicare assignment. Federal regulations require the patient to be responsible for their Medicare deductible and 20% co-insurance. You will be asked to present your card at the time of your visit so that we can make a copy to keep in your record.
The clinic is enrolled in a number of alternate health care groups. Please be sure to follow the specific referral procedures as outlined in each organization. In some cases, without referral from the Primary Care Physician, we are not able to see the patient.
Dr. Raque will gladly discuss any questions regarding fees or method of payment. The price range of the office visit charges depends on the number and complexity of the problems presented and the total time required for evaluation and treatment.
